You went to the urologist. He wrote you sildenafil 100mg PRN. "Take one before sex."
You filled it. Used it weekly. Felt like a transaction every time — pop the pill, set a 60-minute timer, hope the timing lines up.
You thought that was just how ED meds work. It isn't.
There's a different way to take this class of drug, with a different molecule, that's cheaper per year and doesn't require timing anything. Most US patients are never offered it.
The two ways to dose a PDE5 inhibitor
Same drug class. Two different strategies.
On-demand (PRN). You take a higher dose 30-60 minutes before sex. Sildenafil 50-100mg or tadalafil 10-20mg. Effect lasts 4-36 hours depending on the molecule.
Daily low-dose. You take tadalafil 2.5-5mg every day, same time. Steady-state plasma concentration. No timing required. Sildenafil cannot be dosed this way (half-life 4 hours, blood levels cycle daily).
Both are FDA-approved for erectile dysfunction. The daily protocol was added to the Cialis label in 2008 (FDA NDA 21-368).
The math nobody runs for you
Let's compare two regimens at honest US generic cash prices (CVS GoodRx, April 2026).
PRN sildenafil 50mg, once weekly:
- 52 doses per year × 50mg = 2,600mg total
- 30-pack of generic 50mg tabs cash = $25-30, lasts 30 weeks at 1/week
- Annual cost: ~$45-55
Daily tadalafil 5mg:
- 365 doses per year × 5mg = 1,825mg total
- 30-pack of generic 5mg tabs cash = $20-30, lasts one month
- Annual cost: ~$240-360
Wait, that says daily is more expensive. Hold on.
That comparison only works if you actually use sildenafil exactly once per week. In real life, men who get prescribed PRN cycle through:
- Weeks they use 2-3 doses
- Weeks they use 0
- Months they refill expensive brand because they ran out
- Doses that don't work because of food timing or alcohol — counted but wasted
Real-world average for a sexually active 45-year-old on PRN: 2-3 effective doses per week. That's 100-150 doses/year.
Realistic PRN sildenafil 50mg, ~120 doses/year:
- 4 × 30-packs = $100-120 cash CVS
- Or, the same patient at Hims/Roman PRN subscription = $59-99/month = $700-1,200/year
Suddenly the daily Cialis math looks different.
Why daily tadalafil isn't just convenience
The daily protocol has a clinical case beyond not having to time things.
1. BPH symptom benefit. Daily tadalafil 5mg is FDA-approved for benign prostatic hyperplasia (urinary frequency, hesitancy). If you're 45+ and noticing you wake up at 3am to pee, the daily ED dose covers that simultaneously. Roehrborn et al. 2008, J Urol — significant IPSS score reduction.
2. Vascular tone effect. A small but consistent literature on daily PDE5 inhibition showing improved endothelial function, modest BP reduction (~3-5 mmHg systolic), and possible vascular reconditioning. Not a primary indication, but real.
3. The psychological tax disappears. PRN dosing puts a clock on intimacy. Daily dosing removes the clock. For long-term partners, this matters more than the pharmacology.
The metaphor: PRN is a rental, daily is a lease
PRN dosing is like renting a car for each errand. Cheaper per mile if you only drive once a month. Expensive and annoying if you drive constantly.
Daily dosing is the lease. Higher base cost, but the car is in the driveway. You don't plan around it.
For most active 35-55-year-olds with chronic mild ED, daily is the lease. Most US prescribers default to the rental because that's what they were taught in 2002 when Cialis daily didn't exist yet.

When PRN is actually the right answer
I'm not anti-PRN. There are real cases.
PRN sildenafil 50-100mg or tadalafil 20mg makes sense if:
- Sexual activity is genuinely once-a-week-or-less. (If so, PRN is cheaper.)
- You're using it situationally for performance anxiety only, not chronic ED.
- You have a contraindication to daily dosing — uncontrolled hypotension, certain alpha-blocker combos, severe renal impairment.
- You can't tolerate tadalafil's longer side effect profile (mild back/leg muscle ache in ~3-5%, lasts 12-48h — FDA label notes resolution within 48h).
If three of those are true, PRN is the right tool. If none are, you're probably defaulting because nobody offered you the alternative.
What to do this month
If you're already on PRN sildenafil or tadalafil:
- Count your actual usage from the last 90 days. Be honest.
- If you used >40 doses in 90 days, daily 5mg tadalafil is almost certainly cheaper.
- Ask your prescriber for daily tadalafil 5mg specifically. Cite Roehrborn 2008 and the FDA Cialis daily indication.
If you've never been treated:
- Skip the PRN trial. Start with daily tadalafil 5mg from day one.
- Give it 14 days to reach steady state before judging effect.
- If it doesn't work at 5mg daily, add PRN 10mg as needed (this combo is well-studied).

One thing the daily-vs-PRN debate always misses
ED at any age is partially a measurement of vascular health. Daily tadalafil masks the symptom while doing modest good for the underlying biology.
But if you're under 50 and need it daily, do the bloods we listed in this earlier post. The pill is the easy part. What's behind the symptom is the part that actually shortens your life.
