The number that should scare you: 5 years.

Mayo Clinic followed 1,402 men for 10 years (Inman 2009 Mayo Clin Proc). Men in their 40s with ED had a ~50-fold higher 10-year incidence of coronary artery disease compared to ED-free men. The vascular event was preceding the heart event — by years.

ED isn't always a sex problem. For 60-70% of men, it's a vascular problem. The same arteries that fill the penis also feed the heart.

Your dick is your heart's smoke alarm. It just goes off first because the arteries are smaller.

Most ED at 30 isn't psychological.

You'll read "it's probably just stress." Sometimes. Mostly not.

The 2018 AUA guideline review notes that even in young men, vascular causes are common — particularly if you smoke, your BP runs 130/85, or your dad had diabetes.

Stress causes one bad night. Vascular damage causes a pattern.

What to do this week (before you buy anything)

  • Lipid panel — if your LDL is over 130, your arteries are already losing range

  • Morning testosterone — only if libido (not erection) is also gone. Otherwise skip.

This costs less than one month of Hims. It tells you whether your ED is the symptom or the disease.

And about that $59.

Hims sells you generic sildenafil starting at $22/month, often climbing to $50-99 with bundled add-ons.

The actual generic sildenafil at CVS, paid in cash, is $25-30 for the same 30 pills.
